WAIVER FORM 2024-2025

I choose to participate in classes, workshops, trips, tours, events and other such activities held by Talented Kids PAS & MVW Talent Agency of my own free will, and confirm that I am in proper physical condition to take part in such activities.

 

Should I have any doubts of my physical capability to participate in these activities, I will consult my Health Care Provider/Doctor, and inform Talented Kids PAS & MVW Talent Agency in the first instance. Should I have any known physical vulnerabilities, disabilities, conditions or injuries that may impact my ability to participate in the aforementioned activities, I agree to disclose them to Talented Kids PAS & MVW Talent Agency.

 

I willingly release, permanently discharge and hold Talented Kids PAS & MVW Talent Agency (and their directors, owners, students, teachers, staff, employees, colleagues, volunteers, associates (collectively referred to as “Talented Kids PAS & MVW Talent Agency”))  harmless from any, and all claims or demands for:  

• damages, loss of services, costs and expenses, injuries, attorney fees, and any other call for reparation from any bodily injury, personal injury, illness, death, property loss or damage resulting from my participation in classes, camps, intensives, workshops, performances, events, trips, tours etc,  
• the use of Talented Kids PAS & MVW Talent Agency equipment or facilities, 
• any activities held by and/or associated with Talented Kids PAS & MVW Talent Agency.

Despite Talented Kids PAS & MVW Talent Agency taking all safety measures and precautions to lower the risk of injury, accidents may still happen. By signing this waiver form, I/we (the student and parent/guardian) assume all risks related to the use of any and all spaces used by Talented Kids PAS & MVW Talent Agency. Furthermore, I/we agree to obey all class and facility policies and take full responsibility for my behaviour, in addition to any damage I/we may cause to any Talented Kids PAS & MVW Talent Agency utilised facilities. If I have questions or concerns regarding any policies I understand that it is my responsibility to bring them promptly to Talented Kids PAS & MVW Talent Agency attention.

 

Occasionally Talented Kids PAS & MVW Talent Agency classes require hands-on instruction in addition to verbal instruction. Teachers may instruct performers by touching their arms, legs, feet, hips, back and head to move them into the correct and safe position. I understand this is common practice in dance/singing/movement/performance instruction and that it is my responsibility to communicate clearly with my teacher, instructor and/or the director if a form of physical touch is unacceptable to me. Please Note Teachers will ALWAYS ask consent BEFORE any hands on instruction is required.

 

I authorise Talented Kids PAS & MVW Talent Agency to obtain any necessary medical or dental treatment, such as ambulance transport, first aid, hospitalisation, or other care necessary for my health in the unlikely event of an emergency. If my insurance does not cover emergency treatment that Talented Kids PAS & MVW Talent Agency deems necessary, I agree to be responsible for all costs on my behalf.

I acknowledge that Talented Kids PAS & MVW Talent Agency may take photos and/or video recordings of me in classes and activities. I authorise full rights and interest in these recordings to Talented Kids PAS & MVW Talent Agency. I acknowledge that these recordings may be used for advertising purposes in physical or virtual form. In the event that I do NOT consent to being photographed or video-recorded, I acknowledge that it is my responsibility to make Talented Kids PAS & MVW Talent Agency aware and will be proactive about avoiding being photographed or recorded. I understand that this choice not to be recorded may limit my ability to participate in performances that are photographed and/or videotaped.

 

• acts of God, accident, riots, war, terrorist act, epidemic, pandemic (including the Covid-19 pandemic), quarantine, civil commotion, breakdown of communication facilities, breakdown of web host, breakdown of internet service provider, natural catastrophes, governmental acts or omissions, changes in laws or regulations, national strikes, fire, explosion, or generalized lack of availability of raw materials or energy.
